    Hi everyone!
    I'm Jayden, a 4th-year student at the University of Waterloo, and during my final co-op term, I had the opportunity to work with the CND OHT team. When searching for co-op positions, I was interested in seeking roles related to public health and health policy that offered a smaller, yet diverse team of health professionals to learn from.     I joined the CND OHT as a fourth year undergraduate student at McMaster University. Prior to working here, I knew I was interested in healthcare, however, I was missing the applied experience in health settings. During my time with the CND OHT, I was given the opportunity to work on a diverse range of projects from branding and social media to policy review. This position enabled me to develop meaningful connections with community partners, and I have left here having a deep understanding of connected care.     What is the CND OHT:


    Ontario Health Teams (OHT) is an organization whose job is to transform the provincial health care landscape. This group works towards building high-performing integrated care delivery systems across Ontario that provide seamless, fully coordinated care for patients. Ontario Health Teams helps to achieve better outcomes for patients, improve population health, and better value for the province. Their purpose is to reorganize and deliver health care in a different way for our communities.
