Why become an Affiliate Member of the CND OHT?

• Affiliate Membership is the entry level stage towards full membership in the CND OHT.
• Affiliate members are entitled to regular information updates from the Joint Board Committee but will not participate in decision making activities.
• Affiliate Members are entitled to participate in Joint Board Committee/Ontario Health Team education and training at their own cost.
• Affiliate members are entitled and encouraged to engage in service coordination through the Steering and Operations committees.
• Affiliate members may apply to become Members after one year.

How to Become an Affiliate Member of the CND OHT?

Submit an Expression of Interest form. See below for key dates:

• June 14, 2023: Expression of Interest - Open for submission
• August 31, 2023: Expression of Interest - Closed
• October 2023: Vetting and Approval
