The Cambridge North Dumfries Ontario Health Team (CND OHT) is working to improve the wellbeing of our communities. Join us!

This site is one way that the Cambridge North Dumfries OHT will connect with local communities to:

  1. Provide a space for you to learn about the work that we are doing and provide your feedback on areas of work that are most important to you. You can can participate as little or as much as you would like, at times that are convenient to you.
  2. Involve you in designing ongoing OHT engagement strategies to complement this space, including the development of the Patient, Family and Caregiver Advisory Council.

This site will also be a virtual platform as a place to share key Patient, Family and Caregiver Advisory Council documents, information and discussions.

This site includes several forum discussions, quick polls and Q&A . These tools will:

  • Gather your questions and directly respond to you
  • Collect your perspectives and ideas
  • Help us to better understand your needs

    The Cambridge North Dumfries Ontario Health Team (CND OHT) is in the process of establishing a Patient, Family and Caregiver Advisory Council (PFAC), set to launch in September 2022. This is a significant step towards including the critical component of patient engagement within the CND OHT structure, and ensuring the voices of our service users are heard and applied to our work. We aim to engage service users in all relevant OHT working groups and create one-time opportunities specifically to hear from you.

    If you are interested in acting as a Patient, Family, and/or Caregiver Advisor in any capacity (whether as a long-term member of the PFAC or a friend of the OHT to be contacted about one-time opportunities), please complete the Expression of Interest form. Please post any questions that you have about the PFAC here.
