Do you want to learn more about the Patient, Family and Caregiver Advisory Council (PFAC)?

The Cambridge North Dumfries Ontario Health Team (CND OHT) is in the process of establishing a Patient, Family and Caregiver Advisory Council (PFAC), set to launch in September 2022. This is a significant step towards including the critical component of patient engagement within the CND OHT structure, and ensuring the voices of our service users are heard and applied to our work. We aim to engage service users in all relevant OHT working groups and create one-time opportunities specifically to hear from you.

If you are interested in acting as a Patient, Family, and/or Caregiver Advisor in any capacity (whether as a long-term member of the PFAC or a friend of the OHT to be contacted about one-time opportunities), please complete the Expression of Interest form.
