What are Ontario Health Teams?

    Ontario Health Teams are groups of coordinated service providers which work as one team across a specific region. The goal of these teams is to transform care across multiple sectors and providers into a seamless and coordinated system, which can improve the health of populations across Ontario. These teams can include members from primary care, home care, mental health and addictions services, long-term care, midwifery, rehabilitation and more.

    What is the CND OHT?

    The Cambridge North Dumfries Ontario Health Team coordinates care across Cambridge and North Dumfries with 17 Member organizations and 15 Affiliate Member organizations in various service areas.

    What does a Patient, Family and Caregiver Advisory Council do?

    The members of the PFAC will work in a collaborative position with OHT members and partners, to shape health, community and social services. The purpose of the CND OHT PFAC is to increase access to care, improve care coordination and collaboration, and to improve health outcomes and experiences for everyone accessing CND OHT services. This will be achieved by engaging with patients accessing health, community and social services, as well as their families and caregivers. Members of the PFAC will have the opportunity to become involved on various CND OHT working groups to aid in our goal of centering community perspectives, values, and needs. The PFAC members will meet once a month, while working groups have varying meeting times and commitments.

    What is the commitment required to join the PFAC?

    The time commitment for PFAC members is a minimum of 1 hour monthly (for a set meeting, time TBD). Ideally there will be representation from the PFAC on all CND OHT projects, the commitments for which can range from 1 hour biweekly to 1.5 hours twice a year. All members must commit to acting as a PFAC member for a minimum of 1 year, to a maximum of 5 years.

    Who can join the Patient, Family and Caregiver Advisory Council?

    Anyone who meets the following requirements is encouraged to indicate their interest in becoming a PFAC member: 1) You are 18+ years of age, 2) In the past 5 years you have acted as a patient, family member or caregiver, 3) You, or your family member or person you care for, accessed social, community, and/or health services in Cambridge and/or North Dumfries. Beyond these requirements, we will consider those interested in joining the PFAC on the basis of their experience, skills, perspective, interests, and ability to meet expectations.
